eRx (Surescripts)
Search 'Village Pharmacy & Compounding' in your EHR's Surescripts directory, or route straight to our NCPDP ID (eRx routing): 5720265. If the name doesn't come up, try the Miami / 33176 zip — and our team can confirm the IDs by phone.
Fax
Provider fax: 305-200-3783. Include patient DOB, prescriber DEA, and a callback number. Fax orders are reviewed in the same intake queue as eRx.
Secure upload
For practices that prefer a portal upload, our secure upload tools are actively being developed. Open a provider account and we'll set you up with the best current option for high-volume sending.
